When you first arrive please make sure to sign and let our receptionist know of any address or phone number changes. If this is your first visit we have a simple form for you to fill out. We ask that new clients arrive 15-20 minutes early to ensure all paperwork is completed before your scheduled appointment. Visits typically run about 20 minutes, however if radiographs or diagnostics are needed there may be a longer wait.
